    TITLE:

    DESIGNATION OF NINETEEN (25) Q1K BARANGAY COORDINATORS


    IN ALL COMPOMENT BARANGAYS OF THE MUNICIPALITY OF
    CANDELARIA, QUEZON

    I. RATIONALE:

    The Q1K project is a province-wide health monitoring intervention for pregnant


    women and infants that requires sufficient manpower complement in order to
    ensure its widest implementation in all component barangays of the municipality.
    In pursuance of the Memorandum of Agreement between the Municipal
    Government of Candelaria, Quezon and the Provincial Government of Quezon, the
    hiring of Q1K Barangay Coordinators is an essential undertaking to fulfill the
    program’s service requirement, hence this design.

    II. OBJECTIVES:

    a) To expand Q1K’s manpower complement by hiring Barangay Coordinators for


    nineteen (25) component barangays of the Municipality of Candelaria, Quezon;

    b) To assist the Q1K Municipal Coordinator in the implementation of the Q1K


    program in 25 barangays; and

    c) To provide one dedicated focal person in each barangay thereby ensuring


    accessibility for beneficiaries.

    III. METHODOLOGY:

    a) Proper correspondence with Punong Barangays for the nomination of their


    respective Q1K barangay coordinators;

    b) Receipt of nominations and submission of the same to the Provincial Q1K


    Office for evaluation and approval;

    c) Formal notification of approved applications;

    d) Conduct of orientation and training for designated barangay coordinators;

    e) Formal endorsement of beneficiaries to their respective coordinators per


    barangay; and

    f) Holding of regular meetings for monitoring and ensuring program sustainability.

    IV. PARTICIPANTS:

    Nineteen (25) duly designated Q1K Barangay Coordinators

    V. PERIOD OF ACTIVITY AND VENUE:

    For the period covering January to December, 2019; all component barangays
    of the municipality.

    VI. PROGRAM OF ACTIVITIES/FUNCTIONS:

    a) Meetings with beneficiaries;

    b) House to house visitations;

    c) Assistance in organizing beneficiaries for consultations, health examinations


    and sonogram availment;

    d) Filing and review of beneficiaries’ records;

    e) Manning Q1K stations; and

    f) Other incidental activities.

    VIII. FUND SOURCE:

    The honoraria of Q1K Barangay Coordinators in the sum of TWENTY THREE


    THOUSAND NINE HUNDRED FORTY PESOS (₱23,940.00) shall be charged to
    the GAD BUDGET of the MUNICIPAL SOCIAL AND DEVELOPMENT OFFICE .

    IX. EVALUATION:

    The expected result of this activity is the fullest and most efficient and
    effective implementation of the Q1K program of the Provincial Government in all
    component barangays of the Municipality of Candelaria.
