Dear Dr. Edward Lapong,

Warm greetings of peace and prosperity!

I, Rea Michaela Narvaez, on behalf of the 1st-year students of Bachelor of Science in


Agriculture major in Agronomy, am writing to request permission for our upcoming farm visit,
scheduled on December 13, 2023, from 1:00 PM to 4:00 PM at MBX Grapes Vineyard, Brgy.
Tambler, General Santos City. This visit serves as the final requirement for our National
Service Training Program (NSTP) Civic Welfare Training Service (CWTS) course.

A total of 25 students will participate in the farm visit. Rest assured, we will mandate all
participants to submit their parents' consent forms. Additionally, careful monitoring will be in
place to ensure everyone's best behavior, guaranteeing the safe and successful
accomplishment of our activity.

We hope for a positive response to our request. Thank you for your consideration and support.

Brgy. Tambler, General Santos City. I understand that this visit serves as their final
requirement for their National Service Training Program (NSTP) Civic Welfare Training Service
(CWTS) course.

This is to inform you further that I have considered the importance my son/daughter
will gain from participating in this activity, provided that reasonable care and precaution will be
taken to ensure my son/daughter’s comfort and safety, and that I am not holding you
responsible for any and whatever incident that may happen to the prejudice of my
son/daughter which is beyond our control.
