VIII. Beneficiaries
Primary beneficiaries – Brgy. Pantay residents

This project would educate the residents in Pantay, Calaca about the importance
of good nutrition and health. This project will also encourage everyone to eat
healthily and exercise regularly, both of which are necessary for a person's growth
and well-being and maintain the cleanliness of our community.

Secondary beneficiaries – other relatives of our ka-barrios who are currently

residing in a different community

The safety of all people is of utmost importance to us as we deal with a pandemic.

This initiative will teach the inhabitants about health and urge them to eat a healthy
diet, which will enable them to reach optimal health. Then they may educate their
relatives who live in other barangays about the importance of this information. Also,
our ka-barrios might encourage their family members to keep the environment clean
so that everyone could be a valuable part of society on their own.

IX. Rationale of the project

Our community's lack of health care utilization was evident, as practically all of
our respondents stated that they never had medical appointments from the barangay.
As a member of our community, I can see that the reason for this is that there are no
programs in place to check and enhance the health of all people, regardless of age.
Nonetheless, primary health care is critical since it aids in illness prevention and
improves the quality of life. Furthermore, where we live has the largest impact on our
health, including whether we have access to good foods, safe outdoor activities, and a
social network that encourages us to be healthy. As a result, as a young person with a
goal to solve this problem, I intend to raise awareness by distributing a small
magazine or brochure that specifically states the steps that an individual must take in
order to maintain and improve one's well-being, the benefits of health care, and the
disadvantages of not doing so. Furthermore, I will suggest a program aimed
specifically at our Barangay Health Workers (BHW) that will focus on providing vital
services in our community such as screenings, social support, and counseling. This
could make the goal of improving the health-care system in Pantay, Calaca more
visible and achievable.

Another major issue in our neighborhood is solid trash. Individual commitment,

time, and effort are necessary to practice reduction, reuse, and repurposing, which is
seen as significant work. After observing the community and people’s way of life in
Barangay Pantay, I realized that waste management is a big problem. When I asked
the residents of the barangay how they dispose of their trash, almost everyone
responded "thrown wherever," "open-pit," or "burned." I noticed that many areas here
lack garbage cans and bins since their waste is not properly processed and dumped.
Our collaborative efforts in starting a clean and green project, which I advocated for,
can help to improve the target sites. Anyone who litters or throws garbage or other
waste matters in public places such as roads and canals must participate in a
community clean-up drive for a week or pay a fine or penalty money by providing
cleaning materials and trash cans in every area of the barangay and labeling them
with 'Biodegradable,' 'Non-biodegradable,' and 'Recyclables.' I feel that by doing so, I
will be able to instill the appropriate way to dispose of waste in the brains of our
people, in our community.

X. Objectives
o To guide and develop the health awareness of every occupant in Pantay
 Distribute brochures that have information about the benefits of taking
care of one’s health as well as the opposite
 State how to exercise properly
 Guide their food intake by teaching the proper allocation of go, grow,
and glow foods

o To encourage my ka-barrios to strive for the betterment of their health

 Enlighten my ka-barrios about the true essence of a healthy lifestyle
 Introduce healthy but budget-friendly foods
 Encourage them to do group workouts

o To provide non-biodegradable and biodegradable trash cans in specific areas

in Pantay
 Make recycled trash bins
 Distribute to the specific areas that need a proper waste management
 Label them with non-biodegradable and biodegradable as well as how
to distinguish them

o To encourage my ka-barrios to strive for the cleanliness of our community

 Educate them about the negative effects of improper waste segregation
on our health
 Pursue them to make a fresh and safe environment in our community
 Show them a proper example of a person who cares for the
environment, especially through waste management

XI. Description of the project

Individuals' use of health-care services for purposes of preventing and curing

health problems, supporting the maintenance of health and well-being, or getting
information about one's health state and the prognosis is quantified or described as
health-care utilization. Hence, knowing how to take care of one’s health would have a
great impact.

Moreover, maintaining a clean atmosphere is also essential for maintaining good

health. Many disease-causing bacteria, such as those that cause diarrhea and worm
infections, can be found in filthy environments. This means that maintaining good
personal hygiene and keeping the surroundings clean can help to prevent a variety of
diseases from occurring. This led the project leader to make and allocated trash bins
to certain necessary places in our community.

Letter of Request
Hon. Nemencio V. Alvarez
Barangay Captain
Brgy. Pantay, Calaca, Batangas

Greetings, Nemencio V. Alvarez!

I am Mary Antonette N. Mulingtapang, a resident of Brgy. Pantay, Calaca, Batangas

would like to ask a request to your good officer’s assistance in my proposed project.

The project is entitled “KKK: Kalinisan at Kaalaman para sa Kalusugan ng mga taga
Pantay” and it aims to solve a problem within our community. The Brochure and health
consultations and the trash cans in this project will benefit all of the residents of our
barangay in terms of health awareness and cleanliness.

I am hoping for your kind consideration!

Yours truly,
Mary Antonette Noche Mulingtapang

XII. Strategies and methods of implementation

First and foremost, this plan or project proposal should be finalized and
thoroughly completed to ensure that it will guide the duration of the project's
completion. After completing the proposal and receiving permission from the project
adviser, the project leader will collaborate with the Barangay Health Office as the
people of Brgy. Pantay is the intended beneficiary of this project. Upon consultation
with the Barangay Health Office, this proposal should be signed and approved by the
appropriate barangay representative. The project leader will need to travel to the
barangay office to deliver the proposal. If the project is approved and authorized, the
project leader will continue to work with the barangay health office to gather
additional health information to be included in the brochure that would be distributed
to ten representatives from families currently residing in Pantay.

In the following step, the overall budget for the project should be determined.
Because the project leader will be unable to meet all of the financial and material
requirements on their alone, the project leader will need to seek assistance from a
potential sponsor or partner for the project. Project proposals, particularly those
involving the budget and objectives, should be thoroughly explored with potential
sponsors to encourage them to contribute to making this project idea a reality for the
benefit of the community as a whole.

If everything is in order, the project leader will begin collecting information for
the brochure. The project leader will then edit the brochure after that. Following that,
the project manager will start gathering the supplies for the garbage cans (recyclable
items and other materials such as glue guns, wires, and so on). The printing of the
brochures will begin the following day after the collection of materials to be used in
making the trash cans; the printing should be complete within one day. Following the
printing, the project leader, with the assistance of some barangay officials, would
assess the locations that would be prioritized for garbage can distribution. After
completing the assessment, the construction of the garbage cans utilizing the
materials collected would begin. The distribution of brochures to ten heads of
households residing in Pantay will commence the next day. They will also receive a
free health consultation and a healthy snack as a token of appreciation. Brochure
distribution, free consultations, and the distribution of free healthy snacks would all
be complete within five days

XIV. Financial Plan, Cost of the Project, and Proposed Expenditures

This project requires a substantial amount of funding in order to be feasible
and achieve its goals. Potential sponsors or partners could contribute to the
initiative in addition to the funds already set aside by the project's organizers. A
full discussion of the project proposal, including the budget and objectives, should
take place with the target sponsor in order to encourage them to assist in making
this project idea a reality for the benefit of the children in the community. A
sponsor could also offer financial assistance or could provide some of the
materials needed for the project.

Moreover, bond papers, a printer with ink, scissors, a laptop, wires, a glue
gun, and trash bags are necessary for this project.

The costs associated with the project are indicated in the table below.

XV. Monitoring and evaluation Mechanics of the Project

There will be a recording of each health consultation, including shooting

images and films. All of the photographs that the project leader will collect
should be accompanied by relevant captions that summarize what occurred on
that particular day. The project leader would ask for data on the progress of
health problems and whether or not most residents have become more helpful
in dealing with their health problems from the barangay health office. As for
the Kalinisan-project, the barangay officials would be asked about improving
waste segregation at the assessed locations that the barangay officials
prioritized for the garbage can distribution. The project leader would propose
that the health consultation project be carried out on an annual basis because it
has positive outcomes for the health of the barangay's residents.

XVI. Plans to Ensure the Sustainability of the Project

The project’s goal is to guide and develop the health awareness of every
occupant in Pantay, encourage them to strive for the betterment of their health
and make them strive for the cleanliness of our community.

As a result of this project, the Barangay will be able to address the issue of
health in the community. If people cannot afford to access and reach
healthcare units for a variety of reasons, we will bring the healthcare unit
closer to them through this project.

For this to happen, the partner agencies will be involved in maintaining

the project and its current plan of operations, as the project leaders are unable
to meet the demands of this project on their own. Furthermore, because this
project falls under the food, health, and sanitation categories, it will help the
people who live in the barrio. The community of Pantay will benefit from this
initiative since it will teach them how to live a healthy lifestyle while also
promoting the cleanliness of the community.
