1. The Schools Division Office of Nueva Vizcaya (SDO-NV) is pleased to announce

2. All interested applicants shall submit their application letter to their

prospective schools as soon as possible with the following attachments
arranged with Table of Contents and Ear tabs to facilitate an easy access of
documents during the assessment:

A. Letter of Intent
B. CSC Form 212 (Revised 201 7) with two copies of latest 2 x 2 ID
picture
C. Certified Copy of ratings obtained in the Licensure Examination
for Teachers ' Board Examination
D. Certified copy oftranscript ofrecords
E. Certified Copies of service records, teaching experience
F. Certified copies ofCertificate oftraining
G. Voter's ID and/ or any proof of residency
H. NBI Clearance
I. Omnibus Certification of authenticity and Veracity of all
documents submitted, signed by applicant.
